Our services include both physical and mental health. It is our goal to make sure our patients are given the best care possible so that they may have a comfortable and healthy life. Nursing may be the best choice for a patient if he or she has recently experienced:

  • A fall within the past 14 days
  • Changes in medication as a result of a chronic condition
  • A hospitalization or exacerbation of Congestive Heart Failure
  • A recent hospitalization or exacerbation of COPD
  • Hyper or hypotension with recent changes in medications
  • New wound or incision
  • A surgical procedure requiring post-op nursing care
  • AFib diagnosis with medication changes
  • Pneumonia or Bronchitis episode within the past 14 days
  • Recent CVA (cardio-vascular accident) or decline
  • Diabetes Mellitus (especially if patient has had changes or it is uncontrolled)
  • Parkinson’s – decline with recent changes in medications
  • Cancer diagnosis
  • New feeding tube or Foley catheter
  • Unmanageable pain
  • Orthopedic surgery addressing therapy needs with anticoagulant management
